At any time you may remove an affiliation with a care team.

Remove an affiliation
To remove an affiliation with your care provider or team, you must already have an affiliation them first.
From Settings
If you accepted an affiliation invitation from your care team, you may remove it at any time in the affiliation app settings. Here are the steps to remove an affiliation inside Settings:
- Tap the main menu
- Tap on "Affiliations"
- Agree to the terms
- Choose to "Unaffiliate"
